CONSENT FOR THE PROCESSING OF PERSONAL DATA

Declaration of consent for the processing of personal data in connection with the provision of services by Hiruko Sp. z o.o., operator of the Phoenix Europe online store

 

        Pursuant to Article 6(1)(f), Article 7, and Article 9(2) and (3) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons concerning the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC (General Data Protection Regulation – GDPR), published in the Official Journal of the EU L 119/1, 4.5.2016, hereinafter referred to as “GDPR”, I consent to the processing of my personal data provided to the Data Controller, Hiruko Sp. z o.o., operator of the Phoenix Europe online store, in the following scope

1. Contact via voice calls (telephone conversations) for service fulfillment and the presentation of commercial and marketing information.
2. Contact via electronic communication means under the Act of 18 July 2002 on electronic service provision (Journal of Laws 2002, No. 144, item 1204, as amended) for the presentation of commercial and marketing information and service fulfillment.
3. Processing and publication of my image recorded while using the services of Hiruko Sp. z o.o., operator of the Phoenix Europe online store, including taking and processing my photos and video recordings, as well as their distribution for commercial and marketing purposes on the website, social media platforms, leaflets, and other advertising materials for the commercial purposes of Hiruko Sp. z o.o. and its other projects.
4. The withdrawal of consent for the use of my image takes effect from the date of cancellation. The removal of my image from commercial and advertising materials may incur additional costs for the company related to creating new materials or modifying ongoing advertising campaigns.

I declare that I am aware of my rights, including the right to:

1. Request the Data Controller, as indicated in this statement, to:
   • Access my personal data.
   • Rectify my personal data.
   • Erase my personal data if one of the circumstances under Article 17(1) GDPR applies, provided that processing is not necessary as specified in Article 17(3) GDPR.
   • Restrict the processing of my personal data in cases outlined in Article 18(1) GDPR.
2. Object to the processing of my personal data:
   • For direct marketing purposes, including profiling, insofar as processing is related to such direct marketing.
   • For scientific or historical research or statistical purposes under Article 89(1) GDPR due to reasons related to my particular situation, unless processing is necessary for a task carried out in the public interest.
3. Transfer my personal data.
4. Lodge a complaint with the supervisory authority, namely the President of the Personal Data Protection Office, if I believe that the processing of my personal data violates GDPR regulations.
5. Withdraw my consent for processing my personal data at any time.

Furthermore, I declare that:

a) I have been informed that this consent is voluntary and can be revoked at any time by a written statement submitted at the headquarters of the Data Controller, Hiruko Sp. z o.o., operator of the Phoenix Europe online store, or electronically via email at info@phoenix-europe.eu.

b) I have been informed that detailed information regarding the processing of my personal data is available in the privacy policy.
